Registration Process

For the 2023-2024 season, we are using an online registration process. Parental
signature is required for all forms. Please follow the procedures listed below:

Complete (download, fill in blanks, and sign) all required forms. Hand written or Adobe signatures are acceptable.

Required Forms
· FDIHC Registration
· FDIHC Parent Consent for Field Trip
· USA Hockey Waiver of Liability
· USA Hockey Consent to Treat
· USA Hockey Registration

Email the required forms to info@fdihc.org with "Cannons Registration" in the subject line.

Pay the $300 Registration Fee
